Option One: Digital Scans 

For digital scans and 3D printing, download the PDF prescription below and email it along with the .stl file to TheLabGuy3D@gmail.com 

We can accept files from any intraoral scanner that provides an .stl file. 


















Option Two: EasyRx

For our most convenient and efficient submission, if you have an EasyRx account, send us an invite and we can easily add your practice to our list.













Option Three: Paper Scripts

For plaster models, download the prescription below and mail them to our lab at 7044 Judi St, Dallas TX 75252
